What is a Breast Augmentation
with Fat Transfer?
A breast augmentation with fat transfer is a cosmetic procedure that uses your own fat to enlarge the breasts. Essentially, fat is harvested using liposuction, purified, and then injected into your breasts to add volume, improve symmetry, and give them natural fullness. This technique is ideal for those seeking a natural alternative to implants, as it uses your own tissue, avoiding the risk of rejection or implant problems and providing a totally natural feel to the breast.
The procedure has 3 essential parts: harvesting the fat, processing the fat, and then grafting the fat into the breasts.
Harvesting
We typically harvest the fat graft by performing a lipo360 (meaning liposuction to the anterior abdomen, flanks, lower back, and upper back), paying extra attention to create a beautifully sculpted and smooth donor site. All the ab sculpting and contouring is always included! We want our patients to benefit from both the fat harvest as well as the fat transfer.
In patients with inadequate fat deposits in the trunk or who prefer to use other areas, we can harvest fat from just about anywhere – including arms, thighs, lower legs, etc. We take extra time and use small cannulas to make sure that the liposuction portion of the procedure is as world-class as everything else we do.

Once the fat is processed and Dr. Turin is satisfied that the graft is of perfect quality, he will very precisely graft the fat into the breasts with small caliber cannulas to create tiny pockets and strands of fat. Again, the extra care and attention to detail will create better fat graft take and, ultimately, a more beautiful and lasting result.

Generally speaking, implants are excellent for someone who wants a very dramatic shape and size change, like going from an A cup to DDs or something similar. What to Expect From YourFat Transfer Breast Augmentation
Your experience starts long before the actual procedure. Once you’ve had a chance to meet with Dr. Turin and agree on the plan, we will set up your preoperative appointment where you will have more time dedicated to really going into details about your preferences, recovery, etc. You’ll also get Dr. Turin’s cell phone number at that time so that you can reach out to him directly with any other questions. If you’d like to have another in-person or virtual meeting before surgery, we will make that happen for you also – whatever it takes until you’re totally comfortable and in the right frame of mind!
On the day of surgery, you’ll talk again with Dr. Turin and go over all the markings together. We prefer to do the procedure in the comfort of our clinic procedure room, where you’ll have some sedation and mild pain medications to make you comfortable. However, you’ll still be awake with NO general anesthesia. We take pride in providing a very comfortable experience for our patients. There are also a lot of safety benefits to avoiding general anesthesia besides the obvious cost savings.
Moreover, when you’re comfortable and awake, the experience changes; it’s no longer “you’re having something done.” Instead, it becomes us working together to create the changes you want to see. You will feel so much more ownership of your results!
The procedure usually lasts around 2.5-4 hours. The first 30-60 minutes of that to ensure you’re in the perfect mind space and the vibes are just right before we start. Then we go on to numb up all the areas to be treated with a lidocaine solution, and only once that’s fully kicked in do we start the liposuction – and at this point, the lidocaine makes it a virtually painless experience. Once the fat is harvested and the liposuctioned areas look perfect, we process the fat, numb up the recipient sites, and place the fat there gently by hand, using many, many passes and only placing tiny amounts of fat graft throughout so that we maximize graft take and minimize the chances of fat graft necrosis/cysts/lumps. After we treat all the target areas, we close the access sites and place you in a compression garment before letting you go, awake, to recover in the comfort of your own home.
Recovery and Aftercare
Some common things patients experience during their initial recovery period include swelling, bruising, tenderness, and discomfort. The only positioning restrictions we’ll ask you to adhere to are that no pressure be placed on the breasts, so no sleeping on your belly!
You will see a difference in your shape and size right away, and you will see the final result in the breasts in just 1 month!

Is Fat Transfer Breast Augmentation Surgery Right for Me?
A breast aug with fat transfer may be right for you if you meet the following criteria:
- Would like to increase breast size ½ to 1 cup size (remember—the better the fat take, the more dramatic the result! Dr. Turin is able to achieve 80% fat graft take to give you the best possible outcome!).
- Prefer a natural look and feel over implants.
- Have sufficient fat deposits in other areas of the body for liposuction (Dr Turin is an accomplished expert in executing the procedure in low BMI patients, however).
- Are in good overall health.
- Have realistic expectations about the outcomes.
- Want to improve breast symmetry/restore volume lost due to aging or weight fluctuations.
Fat Transfer in Lean or Petite Patients at Koda Plastic Surgery
Dr. Sergey Turin is proud to be one of the few liposuction experts who routinely perform “skinny BBL” or “skinny lipo” procedures. Fat transfer to the breasts in a skinny/lean patient is conceptually the same as a traditional fat transfer scenario. The difference? In a lean or petite patient, less fat is available for harvest, which means it’s MUCH more difficult to harvest gently and smoothly. The problem with liposuction in lean patients is that every little mistake, every tiny error in the liposuction technique, is immediately visible because there is so little surrounding fat to hide it. These cases are Dr. Turin’s specialty, and he has perfected a very different technique for harvesting in these situations. He uses much smaller cannulas and takes extra steps to ensure the donor sites look perfect once done.
Dr Turin routinely performs liposuction and fat transfer procedures in women with BMIs as low as 18. Although the total amount of fat harvested is much lower than in “usual” liposuction cases, the case still takes the same amount of time—it’s just THAT important to us to make sure that you look totally natural after our procedures without any contour issues or divots.
Fat graft take then becomes absolutely critical when so little fat is available, which is why we are so proud of the ~80% fat graft survival with our current systems.
